Registration and Admission
A Centralized Registration Department for all inpatient and outpatient
services is located next to the Emergency Room waiting area. For more information about registration and admission, click here.
Please report to the Registration Center for ALL outpatient services, including the following:
Information for Inpatients
Your Room: Your room assignment is based upon your admitting diagnosis and the bed availability on the day of your admission. Private (single bed) and semi-private rooms are available. Medical insurance usually does not cover the full cost of a private room, therefore we require a $100 deposit from patients requesting private room assignment. If there are no private rooms available, you may be assigned a semi-private room and placed on a waiting list until a private room becomes available. Please indicate your room preference during the time of your admission or notify your nurse after admission. NOTE: All rooms in the Swing Bed Unit are semi-private.
The Medical Staff: The physician who admits you to the hospital is responsible for directing your care while you are a patient here. Your physician will answer your questions about your illness and care plan.
The Nursing Staff: Twenty-four hour nursing care is provided by a professional team of registered nurses, licensed practical nurses, and nursing assistants. A unit supervisor is responsible for directing and coordinating patient care in each unit. A charge nurse is on duty at all times. Please feel free to request a visit from the unit supervisor, charge nurse, or patient representative if you have questions or concerns about your care.
Calling Your Nurse: A button to call your nurse is located
beside your bed. When you press the button, a clerk at the nursing
station will respond by speaking through the intercom system in your
room. Please be specific in your requests for assistance by telling the
clerk exactly what type of help you need so that the appropriate staff
member will be sent to your room. For instance, the clerk will notify
your nursing assistant if you request assistance to the restroom. If
you need pain medication, please tell the clerk so that your medication
nurse will bring it without delay. If your requests are not answered in
a reasonable amount of time, please press the call button again and ask
to see the “Charge Nurse.”
Your Safety: Please use the call button for assistance
before attempting to get out of bed if you feel weak for disoriented.
Strange surroundings and medications may create a hazard if you get up
without assistance. Please do not bring electrical appliances, such as
hair dryers or electric razors, to the hospital unless they are first
inspected by our maintenance department. While you are here, the
hospital staff may have to practice fire or disaster drills. These
drills are held to assure maximum safety for our patients and
employees. Please do not be alarmed if you hear or see evidence of such
a practice drill.
Infection Control: The Infection Control department at
Memorial Hospital and Manor has the responsibility of reporting certain
communicable diseases to the Georgia Department of Health. The
primary purpose of the Infection Control Department is to improve
health and patient safety by reducing the risks of infection and other
adverse events.
Special Communication: Memorial Hospital and Manor has access to an interpreter for a number of foreign languages if a patient should require it. Arrangements may also be made to have a sign language interpreter to assist hearing impaired patients.
Advance Directives
are written legal documents that explain your medical wishes and allow
you to appoint someone to make medical decisions on your behalf if you
are unconscious or unable to make your own health care decisions. For
more information on Advance Directives, contact the Social Services
department at (229) 243-6372, or ask your nurse to make the call for
you.
Personal Belongings: Patients are asked not to bring large
sums of cash or items of value to the hospital. If you do bring a
valuable item, please ask your nurse to deposit it in the safe. You
will be given a written receipt for all items which must be presented
when you withdraw them. The hospital does not accept responsibility for
lost or stolen personal items. If you lose something, please notify
your nurse immediately and we will make every effort to help you find
it. When you are discharged from the hospital, please make sure you
have collected all your personal belongings. The hospital cannot be
responsible for your personal items or valuables.
The loss of dentures, hearing aids, and partial plates is a very common
problem in ALL hospitals. Like other valuables, the hospital cannot be
responsible for the loss of these items. You can help prevent this loss
by making sure your nurse knows you have removed your dentures, hearing
aid, or partial plates. Also, ask your family to help safeguard them when you
leave your room. Do not wrap these items in tissue or toilet paper,
washcloths, or anything else that might accidentally be thrown away.
Chaplaincy Program: An organized group of local ministers is available to assist with your spiritual or emotional needs. Someone from the group is always on call to help when needed. Also, clergy from the area visit the hospital regularly and are welcomed to visit members of their congregation. Upon request, the nurse or patient representative will call a member of the clergy of your choice or the chaplain-on-call.
Mail and Flowers: Mail or flowers addressed to you will be
delivered to your room. Mail received after you have been discharged
will be forwarded to your home address. Out-going mail should be given
to your nurse to be mailed.
Parking: Free parking for Memorial Hospital and Manor
patients and visitors is located in front of the hospital and on the
west side of the hospital, adjacent to the Emergency Department.
Medical Records: Information in your medical record is
strictly monitored and confidential, and may only be released with
written authorization of the patient or guardian. Copies of records are
available for follow-up care at no charge. For other purposes,
including personal or legal use, a
nominal fee is charged. The Health Information Management
Department is open for record release from 8 a.m. until 4:30 p.m.,
Monday through Friday. Patients are required to present a
photo ID and sign a release of information form when requesting copies
of their medical record.
Restrooms: Handicap-accessible restrooms are located
adjacent to the front lobby, Emergency Department, Nursing Home,
Radiology (X-Ray) Department, and on the second floor of the hospital,
at the end of the South Hall near the Intensive Care Unit.
Smoking: In the interest of promoting a healthy and safe
environment for patients, visitors, and staff, smoking is strictly
prohibited inside Memorial Hospital and Manor. This policy is in
compliance with the standards of the Joint Commission on Accreditation
of Healthcare Organizations (JCAHO). Although strongly discouraged,
smoking is permitted only in designated areas outside the facility.
Patients who are receiving IV therapy or who are on fall precautions
are permitted to smoke only when accompanied by hospital personnel.
Assistance to the designated smoking area will be provided as staff
duties permit.
Vending Services: A vending service for snacks and refreshments is centrally available on the first floor next to the double elevators.
